As part of volunteering with us, we ask all divers to complete some forms before participating. This provides information to help us responsibly manage the program. Please download the medical form, fill it out, scan it, and email it back to us.
For the Medical Form specifically: If you answered YES to questions 3, 5 or 10, or to any question on page 2, you must read and agree to the participant statement by signing and dating it, and then take all three pages (the Participant Questionnaire and the Physician’s Evaluation Form) to your doctor for a medical evaluation. Participation in the project will require your physician’s approval.
